COVID-19 Safety Plan
* All clients must use the alcohol-based hand sanitiser provided upon arrival.
* Temperature may be checked upon arrival. If you have high temperature, you will be asked to leave/reschedule.
* All clients coming to an appointment must agree to disclose their details for attendance record.
* All clients must arrive solo to their appointment. Children, partner, family members and friends are not allowed.
* Please dress warmly or bring your blanket as we won't be supplying them. Bring your face mask if you wish.
* All clients who are feeling unwell must stay home. Your appointment will be cancelled/rescheduled if you arrive sick.
* Do not book in if you have had contact with a known or possible COVID-19 case.
* Cash is accepted. Please wash your hand after handling money.

General & Cancellation Policies
* For new client, please spare 15 minutes for consultation and please be aware that we are unable to refill job done elsewhere. Removal is free if you book a new set with us.
* Please remove your eye make up before appointment. Clients who come with heavy eye make up will be charged $20 make up removal fee.
* $50 booking fee will be forfeited for any cancellation / rescheduling / no-show within 48 hours of appointment.
* If you are running late and we are not notified, your appointment will only be hold for 15 mins before your $50 booking fee being forfeited.
* No refill can be done if there is mascara on your existing extensions. It will be removed and a new set price will apply.
* We respectfully request that do not bring your child(ren), partner, family members and friends due to child safety standpoint and Covid-19 restrictions.

Terms and Conditions Waiver Form
* I understand that there are risks of, but not limited to, allergic reactions, eye irritation and/or redness.
* I understand that I have to disclose any allergies that I may have such as latex, surgical tapes, cyanoacrylates, Vaselines etc.
* I understand that I need to remain still with my eyes closed during application.
* I allow my lash technician to use lash shampoo and saline to cleanse my eyes.
* I agree to notify my lash technician if I become uncomfortable at any point during the service.
* I understand that synthetic eyelash extensions will be applied to my natural eyelashes as determined by the technician, and not to create excessive weight on the natural eyelash by preserving the health growth and natural look of my natural eyelashes.
* I understand that bad hygiene maintenance can lead to infections and discomfort.
* I understand that the effects of refill will be different from a new set, as with refill, only empty gaps will be filled in accordingly.
* I understand that I should book a new set with removal after every 3-4 refills to (i) ensure proper cleaning of my lashes, (ii) maintain healthy lash condition, (iii) free from oil build-up, and (iv) help with retention issues.
* I understand that I need to follow the aftercare instructions in order to maintain/extend the life of my lashes extensions.
* I will discuss and confirm the length, curl, thickness and shape that I prefer with my lash technician prior to the session. I understand that extra fee will be charged if I am unhappy with the result and wanting to redo.
* I understand that I may be eligible for a free refill if I have lost 40% of my extensions within 3 days after my initial appointment. No free refill will be offered thereafter.
* I have read the waiver form entirely, and have answered everything to the best of my knowledge.
* I agree that by reading and signing this waiver form, I release Chen Yi Hung (Stephanie) and Chen Chiong Hung (Tiffany) from DANDELASHES from any legal claims or damages of any nature.
* I confirm and agree that I wish to engage the services with Chen Yi Hung (Stephanie) or Chen Chiong Hung (Tiffany) from DANDELASHES.
